EasyDocForms is a HIPAA-compliant workflow tool for independent practices that need intake forms, consents, chart notes, and scheduling in one place. It is designed for practices that want to replace paper packets and manual document handling with structured, mobile-friendly workflows that fit their actual service type, visit type, or documentation requirement.

The product supports form-based intake and charting by letting teams upload PDFs instead of using a complex form builder, then map those forms to the right patient, service, or workflow. It includes built-in e-signatures, conditional logic, insurance card OCR, and support for complex intake packets, body diagrams, ABN forms, outcome assessments, and consent workflows. For documentation, practices can upload a PDF to build a simple chart note template, chart from iPhone or iPad, use supervisory chart review for NP and PA notes, and use AI speech recognition. It also includes a medication barcode reader for scanning lot numbers and expiration dates.

Scheduling is available through the built-in scheduler or by syncing with Square Appointments. Practices can connect forms and consents to specific services or visit types so patients book, complete intake, and arrive ready for the visit. The platform is positioned for forms-only practices as well as full appointment workflows, and it offers custom EMR integration options by request.

Platform details include signed BAA, encrypted storage, and Google Cloud infrastructure, along with separate staff and provider accounts. USA-based tech support and onboarding are available 7 days a week.
